Yes not every time bcz those instances were just obvious in how starship malfunctioned. In this one, if it's not FTS than FAA needs to push for mishap inquiry otherwise musk would push for another one soon. Ultimately this whole flight path is just accident waiting to happen and then it happened

SpaceX says Starship debris fell “into the Atlantic Ocean within the predefined hazard areas."

But FAA told us that it activated a “Debris Response Area” after Starship exploded. Such a step is only taken if debris falls outside designated areas, according to the agency.

I emailed the FAA and specifically asked (1) if they’d be opening a mishap inquiry and (2) if they would, once again, allow SpaceX to investigate itself. Here’s what I got. Looking forward to an updated statement tomorrow.
