I was going through old papers and found the description of my healthcare benefit from my first job out of college.

I paid no premium. The deductible was $100. Prescriptions were $5 for all covered meds. No coinsurance on inpatient or ED care
HEALTH CARE AND FSA PROGRAMS The Firm provides comprehensive medical, prescription drug, and dental coverage. • The Firm provides these benefits at no cost to you. • Coverage is also available for your spouse (or domestic partner) and children at a small cost Medical Benefit Plan Highlights The medical plan allows you to go to the doctor of your choice. You and the Firm receive savings if you choose to see a physician in the United Healthcare Preferred Provider Options (PPO) Program. HMO medical coverage is available in certain areas. Please check with your local office regarding HMOs. The plans pays for 80% of reasonable and customary charges for physician office services, 96% of reasonable and customary charges for surgery and 100% for inpatient hospitalization, emergency care, and annual physical exams. For most services, you must satisfy an individual deductible of $100. The family deductible is $300. There is a lifetime maximum reimbursement per individual of $1,000,000. The plan pays 100% of reasonable and customary charges for: • Well child exams • Childhood and adult immunizations • Annual pap smears and annual routine gynecological exams • Mammograms - ages 35+ • Annual physical exams Prescription Drug Program • Most major U.S. pharmacies participate in the network. • There is a $5 copayment for up to a 90-day supply of covered medications at network pharmacies. • The plan is generous, covering birth control pills and prescription vitamins.
