I have no idea whose name is there or why their service apparently has the wrong person. But it's a screenshot from a chatgpt url. Under different circumstances, id provide a link. But I won't here because I think they're terrible. Here's a screenshot with slightly more information.

Shortly before shamblin killed himself, chatgpt identified that he should be connected to emergency services and then didn't do that. www.cnn.com/2025/11/06/u...

And "this product is reasonably safe" is commercial speech that they knew was a lie when they "said" it. The safety team told them it wasn't ready and they released it anyway to beat Google to market.

There is harm tied to an implied warranty or contract theory that is separate from the ideas conveyed.

A customer purchased a program that provides therapy. There is an implied warranty, that it is fit for the ordinary purposes for which it is originally intended to be used.

Sometimes people say mistake and mean "wrong" and other times they mean "by accident." I think most people who say mistakenly really mean wrongly/illegally. I think it was wrong but not by accident. It was one kind of mistake but not another kind of mistake.
