E2 levels could be the issue then, confirm you're applying them correctly, and maybe ask for a higher dosage. One thing I did on some 2x100mcg patches was stagger them, so switch out one patch every 2 days, which helped with consistent levels.

What are your estradiol levels before and after? Might you have changed patch brand, and it's not working as well? There's all sorts of patch shortages and thus substitutions, and I have noticed differences in their effectiveness.

So the T is likely from the gonads then, so I'm guessing that either something changed with your patches or the P4 is doing the opposite of the usual. You should consider how you're feeling, and if you're seeing remasculinisation. Some options are change the E2 dose/route, pause prog, or add an AA.

If LH remained low, that could be your adrenal glands suddenly producing more T. If LH went from low to not, then the P4 possibly had the opposite of the usual effect on the HPG axis.

I summarise that period during my questioning as a month-long "well shit".
