Under the radar, but big: Trump admin aims to enshittify traditional Medicare. If Medicare denies a claim now, it's because the thing isn't legally covered. The industry denies everything it can, hoping sick folks, faced with all the sludge & red tape, will eventually give up. Also see piece below.

Traditional Medicare historically hasn't required payer pre-approval before patients can access most medications or services, a major perk for enrollees.
