The mimicker of Alzheimer’s disease.

Progressive amnestic syndrome with:
1) Hippocampal atrophy
2) Older age ~ 80+
3) Negative AD biomarkers
• Caution with amyloid positivity common with aging
• Tau PET ideal but not covered
• FDG-PET medial > lateral Temporal hypometabolism = supportive

Additional evidence of brain network dysfunction early on in AD biology - amyloid pathology intrinsically linked to hyperactivity in a vicious cycle.

Each time neurons fire -> APP expressed. Aging neurons with mitochondrial dysfunction struggle with hyperactivity.

This is excellent use of multimodal imaging that is available clinically.

Both Alzheimer’s disease and Lewy body disease can involve posterior regions on FDG PET but LBD by itself doesn’t cause significant MRI atrophy.
