The ONLY time you can call out fatphobia is if someone KNOWINGLY AND WILLINGLY avoid drawing fat characters because they FEAR it will hurt their carreer. However, these kinds of bigots are a minority as most simply never considered drawing fat characters, but will most likely try if suggested.

if an artist doesn't draw fat people that is a consious choice they make every day. it does not matter why they make this choice, the choice itself is fatphobic and is a result of fatphobia. "people who simply never consider drawing fat people" are fatphobes
