Had such a lovely day at Twickenham today. 10s of thousands of people came & celebrated England’s success. We proudly waved the English flag. We celebrated women’s sport. Everyone was welcome. Everyone was kind. We cheered the teams that came 2nd, 3rd & 4th. This is what matters.

Daily Mail previews a "weekend of anarchy".

Calls for mass protests outside 14 asylum hotels appear to seen up to 1500 anti- turn up, with several hundred counter-protestors.

The police made two arrests in Canary Wharf, one from each side. Have not yet seen reports of arrests elsewhere tonight.

A recent pre-print yet to be peer reviewed has been published that explores the very rare side effects of covid vaccine. It’s an important study but one that is quickly being weaponised. I wanted to try and clarify what this preprint does and does not say 🧪

Important thread. Please don’t start regular salbutamol for anything that vaguely sounds like wheeze in ICU unless you are happy it is due to bronchospasm. Appropriate for bronchospasm but entirely the wrong drug for pulmonary oedema and myriad other pathologies that sound like wheeze in ICU #medsky
Rant #1: Salbutamol is poison...

Salbutamol is prescribed freely by trainees and frequently requested by nurses for "wheeze" on ICU and/or as a hangover from initial managment in ED.

It is not a benign drug though and more careful thought is required before prescription

New onset ICU wheeze....

If a critically ill patient suddenly develops wheeze is it more likely that they have developed salbutamol-responsive bronchoconstriction or that we have fluid overloaded them (future rant)?

Is reflex salbutamol prescription appropriate given the potential harms?
