do you ever stare at the ceiling and think about how the worldwide scientific establishment did the impossible and created a COVID vaccine in under a year and the response of the general public has been to go on an unstoppable rampage to destroy science and scientists

People who are saying “oh insurance companies are awful” should do some deeper research into UHG, and in particular into the playbook they run as a “pay-vider.”

It’s actually way, way worse than you thought!

So I'm finally allowed to tell you about what I've been working on for the past year: It's called Offcall.

It's built by physicians for physicians, and we're starting with one of the biggest problems we've heard amongst physicians: salary and workplace transparency.

The vast majority of the cost and risk of drug development is incurred by clinical trials (not pre-clinical work like what’s discussed here).

However, it’d be great if royalties on commercialized products were paid back to grant-funding agencies like NIH!
"But how will Big Pharma do the R&D for new big discoveries?!"

Newsflash: they don't.

Heard about those hot GLP-1 treatments? Yeah, all Big Pharma did was race to patent a bunch of highly similar chemicals, the very last step of R&D.

Y'know who funded the real research to get there?

Taxpayers.

This Thanksgiving, you should consider establishing a codeword amongst your family members to verify your identities over the phone.

It takes <$1 and <3 minutes of audio to convincingly clone your voice into an AI model that can be used by “vishers” (voice phishers) to scam your loved ones 🙂

Just in time for the raw milk revolution!
How many dairy herds in California aren't infected with #H5N1 #birdflu at this point? Another 25 herds confirmed today, bringing the state's total to 461. I would love to know more about how this virus is moving from herd to herd. www.aphis.usda.gov/livestock-po...

A good analysis of the ways that RFK could undermine vaccines (gift link): www.nytimes.com/2024/11/19/h...

One I’d never thought of before is weakening legal protections for vaccine makers

lawsuits, even/particularly frivolous ones, could make them decide hey, this business ain’t worth it
Five Ways R.F.K. Jr. Could Undermine Lifesaving Childhood Vaccines
If he is confirmed as H.H.S. secretary, the longtime vaccine critic would be in a position to change the government’s immunization recommendations and delay the development of new vaccines.

Absolute crisis in primary care… hard to see what fixes this?

Perhaps the Direct Primary Care model?
Nearly 300 primary care physicians employed by Mass General Brigham have notified federal authorities that they want to join a union, citing “burnout” and the “corporatization of medicine” at the state’s largest health system.

The doctors comprise the vast majority of primary care physicians employed by Massachusetts General Hospital and Brigham and Women’s Hospital.

An interesting paper on how different visualizations of different kinds of uncertainty (inferential vs outcome uncertainty) can affect people’s comprehension -> predictions

How much could medical, business, and civic intuition improve w/ better visualization methods? My guess: a lot.

On the meta-discourse of Twitter vs Bluesky, one thing people confuse a lot is the wisdom of hearing people disagree with you versus the danger of hearing people *lying* to you.

A social system needs to have antibodies to actual dishonesty. It distorts reality even if you think you’re immune.

Are there any studies on how much a dollar of primary care reduces consumption of specialty care?

Seems to me this number is probably significant and growing, as specialty care gets more expensive and primary care less uhh… well… “primary.”

What's a recent paper that didn't get enough airtime? For me:

— They found microplastics (MNPs) in 60% of coronary artery atheromas studied
— Those with MNPs had 4x risk of a bad CV triple endpoint and worse inflammatory markers

We are finding MNPs everywhere
