It is scary how frequently we encounter these extremely multi-drug resistant gram(-) organisms in routine clinical practice these days. This Providencia isolate is resistant to everything we tested it against (including salvage drugs not shown)
Antimicrobial resistance is a very real threat.

This study is getting a lot of media attention reports that children exposed to COVID-19 in the womb had slightly higher rates of neuro-developmental diagnoses by age 3. It’s a large study (18,000 births).

This year's #IDweek is now over. Our community has taken so many hits since the pandemic and now the assaults on public health, science, vaccines etc.
Yet the infectious disease community has some of the most dedicated, resilient and thoughtful people I know.

The RSV pre-fusion vaccine Abrysvo is really effective in the real world. In this new study effectiveness in the first season post-licensure, the vaccine was 92% effective in preventing severe infection defined as an ER visit or hospitalization requiring oxygen.

Treated with valacyclovir p.o for 10 days but has progression. Resistance testing shows acyclovir resistant hsv. This is common with hiv coinfection. What are treatment options

Third case person presenting with penile ulcers consistent with previous hsv flares not responsive with oral acyclovir doses he has taken at home.

Ceftriaxone resistance remains rare, dual therapy not recommended, may drive azithromycin resistance. Offer extra genital screening, treat with high dose ceftriaxone.

Case 2: vaginal discharge in heterosexual woman who has had prior gonococcal infections. Treatment status of current partner unknown. How would you treat for gonorrhea here would you worry about resistance would you do dual therapy?

Alternative therapies
Procaine penicillin and probenecid
Ceftriaxone IM daily
But data limited and recommended approach is still IV penicillin.

DoxyPEP can alter serological titers for RPR with slower rise in titers. Since suspicion of otic and ocular syphilis high. Treatment is started.

Challenging cases HIV and STI cases.
First up a serological standoff
RPR 1:4--> 1:8, patient with ocular and otic symptoms. Does not meet the 4 -fold increase threshold for new infection. So do you treat for syphilis now or not?

Patient sadly and unexpectedly relapsed despite combination antifungal therapy. C auris is a very challenging organism to treat and endocarditis frequently fatal.

Isolate was echinocandin susceptible but blood cultures remained positive on micafungin. The panel is not surprised. Hetero-resistance is common and well described with c auris. Other options to treat will be rezafungin or ibrexafungerp. Patient had valve surgery but mech valve 😬

Patient started on broad spectrum antibiotics for culture negative endocarditis. Peripheral blood cultures initially negative but central line cultures grow candida auris.

Fourth case: 39 year old woman in kolkata India with 3 weeks of fever. FUO so challenging and fun.
Started on empiric antibiotics then TB treatment added. Two weeks later fever persist plus she develops a pansystolic murmur.

Third up: fever, gi symptoms, headaches in a young female returning traveller. She has significant travel related exposures including lots of swimming in dodgy waters
