One cool thing about the United States is that there are more people whose job is to deny healthcare coverage without having any medical background than there are people to provide healthcare coverage and they’ve somehow convinced people that the doctors are where the monetary waste is happening
#medsky if an insurer is making a “determination if something is medically necessary”, how is it not a medical decision and therefore practicing medicine?

If you want to understand why healthcare pricing is horrific, the first thing to know is that our system puts 100% of the credit risk for deductibles, copays and co-insurance on hospitals and doctors. That's insane.

We have turned them into Sub Prime Lenders 🧵

There is a human tissue that builds itself up and breaks itself down every 28 days (on average) using non-scarring wound healing and instead of studying that and learning everything we can about it we think it's gross and look for other less relevant models to understand wound healing.

The MITIGATE trial of inebilizumab (aCD19 mAb) for IgG4-RD should get us thinking about which rheumatologic diseases do/don’t warrant deep B cell depletion (i.e. CAR Ts)

⬇️ flares
⬆️ GC-free remission (note rapid steroid taper in placebo group)
⬆️ infections & cytopenias

First-of-its kind evidence in severe dSSc demonstrates that CD19 CAR T may both stabilize/improve:

🔶 Fibrotic organ dysfunction
🔶 Vascular dysfunction (as marked by ⬇️ in RP severity and digital ulcerations)
🔶 Grip strength, contractures, and hand function
