Surely this just reflects 'absence of evidence' rather than 'evidence of absence', given the extremely low incidence of aspiration (1 in 4K in elective surgery).

The authors acknowledge that risk could have increased by up to 1 in 2K cases. A 200% increase in risk of commonest cause of anaes death.
