This is a good thread on NIV.

I’m going to suggest some ways in which my practice differs from what is presented here. I don’t have any RCTs, but we try to consider physiologic plausibility (when has that ever failed?) and outcomes over time.
ICU - Non-Invasive Ventilation (NIV) Secrets:

Did you ever admit to the ICU a patient with COPD exacerbation who came from the ED on NIV? Or who went home on NIV? If you are a pulmonologist, you can skip this post but, for the rest of us from different specialties (I am Internal Medicine),

I’m a HUGE fan of bailing on P:F to some degree in the definition or stratification of ARDS. We talk about Vt or dP being predictors of mortality but then stratify severity by a measure that will change if we put iNO in line (even though the iNO won’t change the outcome).

I'm not sure that's the headline I would be using.

The study wasn't powered to look at clinical outcomes and as all but one patient was intubated on first pass you wouldn't expect to see change in clinical outcomes.

Not every scan is helpful or even do-able. That’s ok!

Absence of sliding can be caused by a number of things, including increased BMI, COPD/emphysema, high PEEP levels, prior thoracic surgery, etc.

Only the identification of a lung point truly rules in PTX (and you often have to look hard to find this!).
