Landmark paper on precipitating factors in acute-onset functional cognitive disorder (ca. 50% of cases).

In line with findings in functional movement disorders, functional dizziness and other functional neurological disorders.

Far from universal but in some FND cases anger is indeed a dominant emotion, not just outward, but also the frustration with the symptoms appears to be a critical perpetuating factor

Objective: Interpersonal conflicts are among the most prevalent stressors before the onset of functional neurological disorder (FND), possibly reflecting maladaptive anger regulation. The authors exam...

New review article on health anxiety for neurologists with Huw Green in Practical Neurology

❌Health anxiety is NOT FND.
✅Can occur in any neurological condition,
✅Reassurance usually makes it worse
✅Specific treatment.

Interesting observations. Wonder if cerebral vasoconstriction could be related to functional vascular diseases such as Raynaud's syndrome, acrocyanosis and erythromelalgia. Much to learn.
OI syndromes in ME or LC: HR & RR are frequently normal despite significant drops in CBF. This often leads to the incorrect conclusion that there is nothing hemodynamically abnormal & thus no explanation or treatment needed. Review CBF assessment OI www.ahajournals.org/doi/10.1161/...

Interesting. If ketamine makes you ignore priors in a way similar to what is observed in schizophrenia, and FND is a problem of abnormally weighted priors and almost no one's seen FND in schizophrenia - has anyone tried ketamine for FND?
New paper in @brain1878.bsky.social: Healthy people under S-ketamine, an NMDAR antagonist, and people living with schizophrenia, a disorder associated with NMDAR hypofunction, spend more time in an external mode of perception - where noisy sensory signals override knowledge about the world.
