The FDA gives Andexxa a thumbs down for full approval. It will be interesting to see what happens next in terms of formulary changes and future research.

1/ Keppra (levetiracetam) loading dose isn’t 1-2g for all status epilepticus patients.

Recommended dose is 60 mg/kg (max 4.5g) as per the landmark ESETT trial.

Important to give full dose especially when case warrants it.

We had this happen right after I started in the Neuro ICU. I teach every resident on my service about the mechanism, so hopefully, it won’t happen again. #pharmsky
ICU Stories:

Middle-aged patient w multiple co-morbidities (CAD/DM2/strokes/seizures/peripheral vasc dz/atrial fibrillation/chronic Foley - UTIs etc) was sent from nursing home to the ED for evaluation of fever/ hypotension. Urine was purulent. CT showed hydronephrosis & bladder wall thickening:

Last week, an AstraZeneca representative reached out to schedule a meeting with me to propose bringing Andexxa to my health systems formulary. Three days later she canceled her request- completely unprompted.
