I think the term ‘knowledge broker’ is an interesting one - to me the ‘broker’ element always has tones of ‘closing the deal’ which might be a bit limiting - what do people think?

Thanks @drlindadykes.bsky.social for drawing attention to the ‘intertwined downward spiral’. Some days that spiral feels like a vortex, from a patient perspective. Spent today arbitraging primary, secondary care and pharmacy trying to get urgent ‘out of stock’ medication. Insulin vials. Failed.
Ironically the success of admission-avoidance schemes is being hamstrung by fragmentation & pressure in primary care (see Rebecca Payne & colleagues’ recent paper) which is also contributing to primary and secondary care being dragged into an intertwined downward spiral. n/3

The BMJ desk-rejected our paper on safety in remote consultations. It later won Research Paper of the Year Award. Now BMJ is publishing a ‘hot pick’ summary of our findings written by someone else. Ah well, so long as the word gets out I guess! @bmj.com

Thanks for this article, Dana. Quite worrying when AI starts inventing its own medical consultation notes, isn’t it? It’s also so difficult to get mistakes on medical notes corrected, in my experience :/
AI can make errors when integrated into healthcare systems.

...but so do humans.

We don't have good processes to correct these errors for humans or AIs.

Some thoughts based on three different clinical encounters as a patient, where my clinician(s) used AI - or not.

I love the ‘advanced technology is just like magic’ quotation - magical ✨ was the word I used at 3am the first night I watched a hybrid closed-loop system do my ‘keep my child alive at night’ job. Then the alarms started going off and it was more like the tech ‘giveth’ and ‘taketh away’ quotation!

Reposting @poemplaynovel.bsky.social request - SugarPixel glucose alarm clocks are REALLY loud - and I think there are bedmats you can rig up. We put vibrating phone on a metal box and it wakes me from next room #T1D and #hearingloss in this house, so really understand your concern!
#t1d help please - anyone have a problem with sleeping through their diabetes alarms?

It’s so loud - I don’t understand how my son doesn’t wake up!

Hoping there is a solution as I’m sure my son won’t live with us forever…
