Description of a prescription cascade, when an initial prescription for pregabalin or amlodipine causes leg oedema leading to addition of furosemide which causes urinary incontinence. This leads to a RX for Solifenacin which causes cognitive decline and a prescription for Don Pascale. The subsequent anorexia and an insomnia leads to mirtazapine prescription and a hip fracture from a fall. This of course leads to many other cascades, including delirium, haloperidol and levodopa, risedronate, and pantoprazole, or iron and constipation with peg.

Yes, MRAs can cause mild hyponatremia. Usually not a major issue but can be a factor. I have not seen severe hyponatremia (eg <125) and use a lot.

Aldosterone deficient mice are mildly hyponatremic (inconsistently)
Primary aldosteronism is associated with mild hypernatremia

Starting next week, Apple’s new hypertension notification feature will be coming to Watch Series 9 and later and Apple Watch Ultra 2 and later with the launch of watchOS 26. That’s according to an emailed statement from Apple spokesperson Zaina Khachadourian saying the feature that monitors blood pressure has been cleared by the FDA. Apple’s new hypertension notifications — announced Tuesday alongside the new Apple Watch SE3, Series 11, and Ultra 3 — use data from the watches’ existing optical heart sensor to “analyze how a user’s blood vessels respond to the beats of the heart.” An algorithm will review this data over a 30 day period and notify users if it detects signs of high blood pressure.

AIM: The “2025 AHA/ACC/AANP/AAPA/ABC/ACCP/ACPM/AGS/AMA/ASPC/NMA/PCNA/SGIM Guideline for the Prevention, Detection, Evaluation, and Management of High Blood Pressure in Adults” retires and replaces the “2017 ACC/AHA/AAPA/ABC/ACPM/AGS/APhA/ASH/ASPC/NMA/PCNA Guideline for the Prevention, Detection, Evaluation, and Management of High Blood Pressure in Adults.” METHODS: A comprehensive literature search was conducted from December 2023 to June 2024 to identify clinical studies, reviews, and other evidence performed on human subjects that were published since February 2015 in English from MEDLINE (through PubMed), EMBASE, the Cochrane Library, the Agency for Healthcare Research and Quality, and other selected databases relevant to this guideline. STRUCTURE: The focus of this clinical practice guideline is to create a living, working document updating current knowledge in the field of high blood pressure aimed at all practicing primary care and specialty clinicians who manage patients with hypertension.
