In another refinery incident there was a circulation pump located just under the white circular vessel you see in this image that failed. From the description of the event and the location, I’d take an estimated guess and say pipeline failure or some sort of flange failure.

Aftermarket solutions to problems that shouldn’t exist is, unfortunately, a familiar process.
To reiterate an opinion I’ve expressed many times, if I have to use someone else’s stuff to get rid of your trash as a manufacturer, stuff that you can block if you care, you have failed me as a customer. In fact, I assume I’m not your customer.

but fr you can detect hex chrome at like below half a microgram per cubic meter and it doesn’t really get “dangerous” until you have chronic exposure at something like 0.005 milligrams per cubic meter and the action level is 0.0025 mg/m3 which is 2.5 micrograms/m3

This is my semi-regular reminder that the intention of Prop 65 was to make sure you knew materials were present that should be disposed of in a way that they wouldn't contaminate the groundwater, at which point you increase your odds in the cancer lottery.
