Germany’s healthcare system isn’t a good example.

#M4A
Germany is a good example of very high costs.

% of GDP
1. U.S. 17.2%
2. Germany 12.3%
OECD avg 38 9.3%

Health expenditure per capita in USD PPP
1. U.S. $14,885
4. Germany $9,365
OECD avg 38 $5,967

The idea that making Americans choose their own healthcare will lower costs

is like saying you should decide & fund bridge safety yourself.

In reality, it just cuts spending because people can’t afford care & also have difficulty choosing the best options.

Medicare reimbursement is too low for Medicare For All?

No or barely.

#M4A
~27% hospitals experience Medicare margins worse than -10%. This implies, even allowing for weak cost accounting & inefficient expenditure patterns, a minimal adjustment in Medicare payments would be sufficient to bring reimbursement to full cost coverage for essentially all hospital Medicare care.

Myth. All can choose costplusdrugs.

2003–2022: Medicare was barred from negotiating drug prices (Part D “non-interference” rule.)

2022 Inflation Reduction Act 1st time Medicare was allowed to negotiate.

2026, limited set of high-cost drugs phased in.

Medicare negotiation is new & narrow.

Perpetuating a myth.

#M4A

Hosp say “Medicare underpays” to defend markups.

Private plans pay ~250% of Medicare, driving prem + OOP up.

Facts:
>50% hospitals profit on Medicare
~33% on Medicaid
~9% profit on charity

Cost control, not rates, explains wins at Medicare rates v losses.

Earth revolutions.

How did they know that?

4.543 ± 0.002 billion years comes from Pb–Pb dating of the oldest meteorites (CAIs), using the U-238→Pb-206 & U-235→Pb-207 decay ratio, with results verified across many samples & methods.

I want to congratulate Planet Earth on successfully completing 4.54 billion revolutions around the sun thus far.

Way to go, chap.

~1/3 of National healthcare expenditures (NHE) is Medicare, 65+.
(Current taxes)

~1/3 of NHE is 45-64yo.
(Can be paid by increasing tax only on the top 10%)

~1/3 can be payroll taxes ONLY on employer side in place of the premiums they pay now.

This equates to no new taxes on the lower 90%.

Health Insurance ≠ healthcare.

Insurance doesn’t “give” anything. It indemnifies loss.

It is best without profit & as broad as possible: Universal / National level.

“Gatekeeper” UHI:
✅ ↓ cost
✅ ↓ admin
✅ ↓ tests
✅ ↑ PCP quality
✅ ↑ equity
✅ = / ↑ outcomes
❌ ↓ speed

U.S.
❌ ↑ cost
❌ ↑ prices
❌ ↑ specialist use
❌ ↑ fragmented care
❌ ↑ gaps
✅ ↑ speed of access
(if insured & not the 1/3 already with “gatekeeper”)

The UK is very different from Medicare. In Medicare you can mostly go to a specialist of your choice. Not in the UK

This is how most UHC works. Will Americans be ok with this change to Medicare ?

Poverty, concentrated wealth & inequality used to be natural fate.

Now they are optional.
(Because of policy choices)
Poverty is a policy choice. Concentrated wealth is a policy choice. Inequality is a policy choice. None of it is natural or inevitable. Remember: We have the power to build a system that serves the many, not the powerful few.

There is a simple plan.

~1/3 NHE is 45-64yo.

Lower Medicare to age 45yo over 5 years by increasing income taxes on the top 10% incomes.

Someone making $300k would pay ~$6k per year extra tax.

#M4A
5-year plan to lower to age 45yo

1: 63-64
2: 60-62
3: 55-59
4: 50-54
5: 45-49

1&2: top 1% 4% add. income tax, next 4% 2% tax, next 5% 0.8% tax.

3: top 1% 6% tax, next 4% 3% tax, next 5% 1.2% tax.

4: top 1% 8% tax, next 4% 4% tax, next 5% 1.6% tax.

5: full add. income tax rates (below)

Medicare For All

Estimate to lower Medicare age to 45yo are $825B more Govt spending (taken away from employer costs) per year in today’s $.

Combined,
10% tax on top 1%
+
5% tax on next 4%
+
2% tax on next 5%
=
$775B
(The $50B is $100 per person per month OOP 45-64yo; employers could pay)
