The only solution to this problem is to drive profit out of healthcare delivery. In years past, non profits and churches used to run rehabs but now most are owned by private equity.

They will refuse to bring patients to follow up specialist appointments after discharge, unless family transports or pays out of pocket.

Medicare pays a fixed amount to these SNFs for each patient— so the only way they make money is by providing substandard care.

Patients who need help with eating and drinking will starve and become dehydrated as sicker, acute patients require more attention. They routinely delay transfer to hospitals until patient is too sick or family insists.

I do not believe staff intentionally neglect patients but one nursing assistant for 15-20+ patients is an impossible ratio. Non ambulatory patients sit in their waste for hours, leading to severe skin breakdown and large wounds, which become secondarily infected form stool contamination.

I try to steer my patients against rehab, towards home care, unless there’s absolutely no other option. I would never recommend anyone I know to go to a rehab hospital that is privately run.

I am an infectious disease doctor and I spend 90% of my time taking care of sick hospitalized patients. Every single nursing home is understaffed and poses serious risks to patients.

This is nonsense— I came to America at about the same age and on my college application, I checked “American Indian”, later noted my place of birth. I know better 20 years later.. but at that time I did not and many immigrants are caught off guard by these choices.

CEO pay is up 1,085% since 1978, while typical worker pay is up just 24%.
