Flights 4, 5, and 6 were all 100% successful. Flights 7 and 8 saw the boosters being caught but the ships being lost. No, "each one" hasn't exploded.
Also, Apollo 6? That's considered a failure, and NASA decided to to ahead and fly crew on the very next flight. Interesting how that wasn't included.

Definitely could've been designed to look better, but it isn't really half bad.
