2/ Tophi frequently occur in the medulla, and usually consist of central regions of acellular, granular material surrounded by epithelioid macrophages, syncytial-type giant cells, and lymphocytes.

Lobomycosis

-caused by the noncultivable fungus, Paracoccidioides lobogeorgii (formerly Lacazia loboi)
-Found in Amazon basin
-causes keloid-like lesions
-Dx: histopath (round yeasts like pearls)

2/ Histologic features of Sarcoidosis typically include well-formed non-caseating granulomas composed of epithelioid histiocytes and multinucleated giant cells along with an active tubulointerstitial nephritis typically composed of a lymphoplasmacytic infiltrate, rarely containing eosinophils.
