In a commentary in Vaccine, a group that included many of the ACIP panelists that were ousted in June used a policy maturity assessment tool to compare ACIP's September meeting of new members against the April meeting, when the former panelists were still serving.

Although you should come to this paper for the grave warning based on the authors' detailed measurement of how ACIP is functionally circling the bowl, you should stay for the incredible evidence-based academic zingers alone.
Once a leading authority in US public health prevention policy, ACIP has experienced a decline in credibility following a series of missteps and a shift in emphasis from vaccine policy development to concerns about vaccine-related harms. The chair repeatedly stated that their activities would “restore public trust in vaccines,” while many on the committee are personally responsible for sowing that distrust. In response, professional societies and state agencies are collaborating with insurers, pharmacists, and other stakeholders to reinforce national vaccine policy and infrastructure. Amid this situation, the public is confused, concerned, and unsure of where to turn for valid guidance regarding vaccines [72,73]. The danger of this moment is substantial. Our collective failure will be measured in lives lost or permanently altered by infections that could have been prevented or mitigated through vaccination. We identified four shortcomings that now hinder ACIP's capacity to develop effective national vaccine recommendations. ACIP has long been known for its scientific rigor. However, the last two meetings were both marked by procedural chaos and low-quality recommendations [74]. Presentations were of variable quality, with CDC experts presenting well-organized and scientifically sound information, and non-CDC experts presenting unvetted information of low scientific rigor or personal anecdotes. The latter presentations highlighted unfounded claims often used in vaccine misinformation and overly focused on already well-acknowledged limitations of vaccine safety studies. In contrast, the limitations of vaccine adverse event reporting studies—including the challenges in establishing causal associations and the relative rarity of such effects—were overlooked, hindering a balanced and informed policy-making process. We surmise that the only working group to convene in preparation for the September meeting (COVID-19 vaccines) no longer used the EtR framework or GRADE analyses. This aimed to advance a predetermined agenda, contributed to vague and imprecise voting language, added uncertainty among the members about the motions they were asked to approve. The ACIP chair challenging former CDC directors to a debate misrepresented the scientific process, which relies on methodical peer review, replication, and consensus as the gold standard for evaluating evidence. The chair calling the new ACIP members “rookies” underlined their inexperience with the process and the damage done by removing the ACIP members and leadership en bloc. Reliance on low-quality evidence was apparent in two ways. First, several voting members were not able to interpret the full body of presented evidence. For example, in discussions on the birth dose of hepatitis B vaccine, one member focused on minor side effects, such as feeding and fussiness. Another member argued that only evidence showing no harm should be accepted, rather than evidence demonstrating absence of harm—contradicting fundamental principles of scientific inference and applying the logical fallacy “appeal to ignorance,” which focuses on what is not known or can never be known. Additionally, several members cited international practices without adequately contextualizing differences in population demographics, healthcare infrastructure, and epidemiology, leaving uncertain the relevance of such comparisons to US policy. Second, repeated mentions of unsubstantiated vaccine safety claims relied on weak outlier studies (mentioned by committee members rather than from presentations), personal anecdotes, and logical fallacies (e.g., appeal to authority, appeal to hypocrisy, hasty generalization) to argue in favor of personal biases and misleading ideas. The September 2025 expansion of ACIP's membership roster increased the diversity of experience relative to the June 2025 committee, but limitations related to in-depth vaccine science and primary care expertise continue to be a concern. What the current voting members have in common is a history of being COVID-19 contrarians, many of whom spread falsehoods about the disease and vaccine during the pandemic. Real-world clinical and public health experience is critical for understanding nuance in translating policy to practice. Beyond policymaking, the current ACIP demonstrated limited understanding of health care implementation, government programs for vaccines, and legislation. While public health policy affects individuals, the ACIP charter is to limit the spread of disease in populations. Individualized health care occurs at the point of care in medical settings, so ACIP must create policies that providers can implement in a variety of circumstances under diverse state rules and regulations.

I really enjoyed this paper because it never deviates from reviewing the evidence objectively & seriously, in the context of the standardized "maturity assessment" tool they used, and at the same time it is a scathing review of the new ACIP's performance.

Got my #Novavax #Nuvaxovid COVID vaccine this morning. I had my #FluShot a couple weeks ago when offered one at work. The CDC officially adopted ACIP's recommendations today, so if it's been hard to get COVID shots where you live due to alignment of state law with CDC recs, try again this week.

5/ Speaking of Covid, after two weeks of waiting, the acting CDC Director signed off on ACIP’s recommendation for Covid-19 vaccines for everyone over 6 months. This usually happens within 24–48 hours of the ACIP vote. That signature now allows state and federal programs to order doses.
