Good question. I think they are referring to concerns that the benefits may not be worth the side effects, logistics, and cost - a barrier to increased uptake in the VA system.

2- Earlier studies demonstrating robust effects of single-dose ketamine may have due to unblinding and expectancy bias.

This study used IV midazolam rather than saline, a better comparator for demonstrating true ketamine effects.
