Lillian got one-shot in the Cazador fight, here's how it played out from my POV.
After drawing his aggression, Lillian takes a substantial bite from Cazador during his boss fight. Hands come into view as Lillian loses blood. Gale is in view as Lillian is about to lose consciousness. Gale: The position of resident blood sucking parasite is occupied. Stay awake. Gale pours a powerful healing potion into Lillian's face, as Astarion and Karlach finish off Cazador in the background.
