I have not - have tried to avoid this scenario and push for PTC if ERCP not feasible. That said, from speaking to colleagues who have operated post-procedure, it’s doable and usually not too terrible. But have your “terrible Whipple” playlist available…😉

1. Mitigation of fistula in elevated risk patients w extended abx.

2. When CR-POPF does happen, drain drain drain! I’ve never re-op’d but results are generally poor.

This study affirms an important point - patients with CRCLM who undergo resection will have better survival than those who don’t undergo resection. Imperative that ALL patients with CRCLM are evaluated by an HPB surgical oncologist.
