We can still lift the flag of measles elimination. A flag held by Americans like Ciro de Quadros and Walt Orenstein to lead the world. Behind which we have achieved massive reductions to childhood suffering around the world.
Or we can retreat and trample our achievement into the sand.

The United States is close behind on this path. "Elimination status" is, on its own, just a label. But then so is "Democracy" or "America". I don't favor military analogies, but there is a reason the flag bearer leads the charge, so those fighting behind know why they sacrifice.

Canada's loss of "measles elimination status" means very little has changed from yesterday. But it means that tomorrow is likely to be progressively worse. More children will get sick at an increasing rate. More schools will be closed. More lives disrupted. And eventually, more children will die.

Like so many things with delayed impacts, by the time you know there is a consequence, it's very late. By the time you know there is a consequence, those who sounded the advance alarm are hoarse from shouting.

This is an embarrassing loss for the Pan-American Health Organization, which had set the global standard for measles and rubella elimination. My own country is sadly close behind. We have done a disservice to the vision of Ciro de Quadros.

I almost edited "feeling of accomplishment" because it betrayed too much of my own pathology. But I've been this way for half a century. There's no hiding from it now.
