Since then it was discovered that certain instruments had some issues. One of the Black Hawk’s altimeter gauges was likely underestimating height, and its ADS-B equipment was likely not active. Those factors could have increased uncertainty around its position and muddled flight communications.

Since some NTSB findings for the crash are now available, I summarized some factors in a root cause analysis.

4 main factors made the situation more risky: aircraft setup, personnel situation, airspace, and sensors. Even in January, the airspace and ATC situation were known to be challenging.
