Physician, runs 100 milers for fun, stage 4 renal cell cancer survivor, wife, mom, and grandma. Opinions my own. No, I won't diagnose you or contradict your physician, so don't DM the ask.
That moment when you DC your last CDA and get ready to head home after working the weekend. #eMedSky

Last night we had a vacationing Aussie roll in. Pt disclosed she was prescribed Andrefeme 1, a "female version" (pt's description) of 1% topical testosterone for post menopausal symptoms. You never know what you'll learn in the ED!

Do not shame the rest of us by being a pompous ass.
Triage rant: just because you’re a physician doesn’t mean you get to skip the ER waiting room. It should mean you respect the triage process even more and understand that if you’re waiting, you’re doing ok.

Hey #MedSky, @weatherwest.bsky.social is a great follow if you want to see a science communicator on #ClimateChange and #Weather #wx in action.
Signs of some "big time" weather for TBD portions of U.S. West coast over next 10 days. A major #AtmosphericRiver, & perhaps a short sequence of very wet ARs, will affect some section of coast between NorCal & WA--perhaps bringing very heavy rain accumulations. #CAwx #ORwx [1/6]

Last month, the so-called American Academy of Sciences and Letters gave a medal to Dr. Jay Bhattacharya, who advocated for mass COVID infection. NY Post framed it as a “top award.”

We looked into it. AASL is 2 years old—part of a right-wing dark money network trying to transform elite universities.
The Invisible Hand: How Dark Money Is Inventing Prestige for Right-Wing Academics
The Academy of Sciences and Letters sounds prestigious, but is it really?

Measles requires 95% vaccination rates for herd immunity.

It has a 16.2% case fatality rate for unvaccinated children under 5 years and 24% for children under 9 months (who are unable to be vaccinated).

30% of the survivors experience severe complications like blindness, deafness, or encephalitis.
These are the vaccination rates of kindergartners by state. It takes 95% coverage of MMR vaccine to prevent measles outbreaks & most states are not achieving it. I fear that under the new administration rates will drop even further and we will start to see preventable illness, disability, and death

The preliminary sequence from the H5N1 human case in British Columbia has been posted and it is not good news. The virus potentially has a quasispecies at HA residue 226 (H3 numbering). This is bad news because we know that mutations at residue 226 can increase binding to human receptors. 1/

Link for anyone interested, it’s the rule that if you make less than $58k you’re eligible for overtime pay.

This rule allowed me to get a salary bump this year because my employer didn’t want me to be eligible for overtime. I’m furious it’s been struck down.

Why would vaccine manufacturers risk making them for the USA in this environment? This would effectively reduce the numbers a vaccines easily (and cheaply) available here.
