I think it's mostly the accent that comes from films? The thing with Pirates having disfigurements and injuries definitely existed in pre-film era books like Treasure Island and Peter Pan. I'd imagine the eye-patch thing was just part of authors associating disfigurement = different = bad guys.

I can confirm that Halloween isn't as big a thing in the UK and Ireland as it is in the USA. Most of our idea of it is spill-over from US media and it's not a big thing for adults here ime.
