Bluesky has a setting one can enable to automatically receive a reminder to add alt text to images. Having the capacity to use alt text and deciding not to is a deliberate choice.
There are bots that will fetch alt text and a feed (Alt4Me) for users with their own accessibility needs.

Alt text is very helpful for a lot of people (myself included) and there's an option to turn on a reminder to include it in the settings. 🖤
