Ofcom investigates BT and Three for failing to connect 999 calls.

Previously, BT fined £17.5m for "catastrophic failure" of emergency call handling service. Three was fined £1.9m.

Execs face no penalties. Fines passed to customers. Execs free to game the system, boost profits and exec pay.
