OK. But how long will you do it ? Non ID doctors raised on the older names will have a hard time memorizing the new ones. The message for my hematology team is "if you see a funny name in BC, check with me or micro" but that may still result in lost time before appropriate empirical Rx

We seek quickest efficacy. Increase to 10mg/kg and monitor renal function/electrolytes. decrease if toxicity. Posa tablets are easier to reach high levels. I'd target trough close to 2, and at least >1.5 mg/L.

Same for empirical Rx. Colistin another empirical option except for naturally resistant enterobacterales. Cefiderocol, fluoroquinolones or tmp/smx if S (~ 30% in my center). Tricky for mero-S strains (in vitro vs in vivo ?) Might try it for non severe infections, with initial combo.
