To be honest, re: engagement on medical topics on X - it has plummeted since Musk took over and it felt anything I was interested in was ignored whilst the toxicity got amped up. I reckon many people even avoid leaving any trace on SoMe even if they may observe from a distance, every now and again.

I think it reflects a wider trend both of fragmentation of social media presence & medics retreating from SoMe in general. Different platforms have stepped up post-X including bluesky, linkedin, instagram but I find the times are very different and enthusiasm with SoMe has plunged.
