Exactly this. It's a one-way street. There is no undoing the damage their thumb twiddling causes.

One of the most depressing things is the cost of the amazing B-cell depleting therapies is actually LESS than some older meds (~$80k/year versus $100k+), and yet they still require step therapy.

"Step up" is unfortunately common with other chronic conditions, too.

With Multiple Sclerosis, there are no cheap, small molecules. All the meds are wildly expensive. Insurers often require failing on older therapies first.
