Although unconfirmed, I think the villagers have built their own villages. The reason why they don't build in the game is for gameplay reasons and not lore.

I feel like cave 14 should tie into some eerie boss mob that lives in an abandoned shrine where it's been sealed away and maybe it's been breaking at the seal for centuries. Maybe the player has to place the Totem of Undying to break the seal to defeat this sealed boss once & for all.

Have you ever thought about how closely similar the Phantoms are to vultures🤔 They both swoop down, fly in circles. I think Zombies and Skeletons would be in fear of vultures and Phantoms attacks the player. So I wonder if Phantoms would get a redesign to match vultures 🤔
