So, because the image is generated from that noise, you also don't have a concrete light source. This image, does. It is mostly bright. AI, generally, does not does that.

That being said, I agree that it looks altered. The smile is what looks it for me. Everything else may well be real.

It seems you don't know the difference between AI and photoshop. And that's ok. Let me give you soem tips:

If it was AI, (which to my belief it is not) the best way to tell is if the shading and lighting was off. AI makes images from noise dark and light. It comes out evenly, looks unnatural
