Answer: A, 5.7-6.0%. A hemoglobin A1c measurement of 6.4% is not awful, but it is an indicator of prediabetes. 6.5% or above is an indication of diabetes or lack of glycemic control in a known diabetic. >7% is not good. Two decades ago, 7% was considered normal.

Answer: D. Prodrug tamoxifen requires CYP2D6 activation for effectiveness. Paroxetine inhibits CYP2D6. This interaction blocking the conversion of tamoxifen to its active metabolite, endoxifen, reduces the efficacy of tamoxifen, increasing the risk of breast cancer occurrence.

Answer: B. Golimumab (Simponi, Simponi Aria) is the missing TNF-alfa inhibitor. Anakinra is an IL-1 antagonist; natalizumab is an anti-α4-integrin antibody; rilonacept is an interleukin antagonist. All of them modify the immune system, but only golimumab is a TNF-alfa inhibitor.

Answer: D. Phenylephrine is an alpha-adrenergic agonist, a vasoconstrictor–orally it is not effective, but intranasally it relieves nasal congestion due to cold or allergy. The ophthalmic preparations relieve redness. It is administered IV for hypotension due to anesthesia.

The answer is B, benazepril. All ACE inhibitors may cause angioedema. Although on average the incidence is less than 1%, it occurs 5 times more often in patients of African descent.

The answer is C, both. ACEIs (lisinopril), ARBs, and potassium-sparing diuretics (amiloride) tend to cause hyperkalemia. Ringer’s lactate solution should be avoided in these patients if possible.

The answers are B, 1, and 3. Aclidinium (Long-Acting Muscarinic Antagonist) + formoterol (Long-Acting Beta-2 Agonist) is a bronchodilator combination used for COPD. It is NOT indicated for asthma or acute bronchospasm.
