I think the key is to treat the internal barriers as equally valid and important to the plot as external ones, which a lot of people... don't know to do, because our culture doesn't think of mental/emotional hangups as "serious" compared to external Plot Factors.

You have to have something that is believably preventing the leads from getting together, AND you have to have an equally plausible solution to that problem. The more believable the obstacle, the slower the burn can be, BUT the reader needs hope that the problem isn't totally insurmountable!
