It's important everyone over 14 on their doctor's learning disability register has an annual health check.

They can help you stay well and find any problems early.

If you haven't been offered one you can ask your doctors practice if you can have one.

Cheers too V&S Project. Being part of the creation of the toolkit was such a special privilege,co production at its best. Whatever you do , give it a try . It can feel tricky to think & plan about the future, but this toolkit will support people with a learning disability to be fully in control 💜
