I suppose Mal also finds unkempt, dirty people repulsive. And not in the 'I've been out working in the fields and got really sweaty' way. But more so those who have the means and yet don't put in the effort to appear at least presentable. So, I guess it all boils down to laziness.

She can tolerate a weeping, sniveling coward if they're at least useful to her in one way or another, but if they're in her way and Mal can't benefit from them somehow? She wants them out of her sight.

And while physical features definitely play a role, it's often personalities that evoke the strongest feelings from Mal. Especially people who break down easily or quickly show that they are weak & defenseless that disgust her. But above all stands one descriptor—useless.
