Nevertheless, there should 100% be a record kept of reasons for wanting to leave and suggestions for improvement - which could be collated across deaneries/country to spot, fix and monitor issues. It would be so quick and cheap to implement. No need for McKinsey!

I love this idea - but I sadly have zero faith that there would be any capability, or even will, to act on suggestions. There needs be overhaul from the top.
