I absolutely agree. However, that is not what we have here. The CMS penalty for being “less than average” for securing organs has created a disturbing incentive for the agencies approaching families.

I took a hiatus and am pleased to see more familiar names here. But I still can’t find the rich discussions that were on the social media site formally known as Twitter.

Whether that’s because I can’t find them, people are too busy/burned out, or having the discussions elsewhere is unclear to me.

We don’t need more poorly designed checklists, alerts, policies, cognitive debiasing exercises, RCAs, and the list goes on.

If you want to improve the safety of healthcare we need to:
1) stop focusing on human error, a post hoc social construct assigned based on hindsight and outcome bias,
2) truly study how healthcare is delivered (RIP AHRQ),
3) partner with people who are experts in the science of safety
