💬 45% of patients with LTCs do not have an agreed care plan to help them manage their condition.

The Government’s recently published 10 Year Health Plan shares their intention to increase the use of care plans stating, ‘by 2027, 95% of people with complex needs will have an agreed care plan.’

🔈22% of patients with LTCs reported they were either ‘not very confident’ or ‘not at all confident’ in managing their condition.

💬50% of patients with LTCs are not having a conversation about how to best manage their condition with a healthcare professional.
