The fundamental issue is that here, free time is free time. No need to keep phone on or anything.

This (Dutch) news article says that many experienced engineers have left the company due to Musk's antics, so the remaining are either inexperienced or pulling double shifts, or both, which would make "human error" true but much closer to your original jab at Musk. 1/2
