Always forget how dystopic Shostakovich’s 5th is. I’m partial to Leningrad so I don’t listen to 5 as much as I should and to this day have not listened to his 4th which was somehow less acceptable to The Regime than his 5th 🤯

I do think acknowledging uncertainty is also important. Allows for better SDM and patient focused care, even if we don’t have good numbers.

Yah. Even when we have A number (CHADS-VASC, surg risk strat scores, bleed s/p fall on apix, etc.), IRL, not as simple as plucking the number. But I guess approach useful. I will quote recurrence of PJI, osteo, etc. that are averages and then be “but I think higher/lower for you bc XYZ”.

But how does one memorize numbers (or even look them up, if they even exist for each patient for each permutation… practically impossible). So the last block is really the one that feels most adjustable.

I have a patient right now, almost certain he doesn’t have TB retinitis, but I don’t have numbers- just no epi risk, negative quant etc. we’re also bad at pegging a pre-test probability. Been a few papers about how poorly doctors estimate probability ie. Acute MI given positive trop etc.
