Yesterday I started building an open clinical terminology from scratch. Of all the projects I've done that have seemed mad/impossible, this seems like the most mad/impossible.

A really important point about excessive US control of digital tech in a Trumpian era, made well here by Cory Doctorow.

This has huge implications for the NHS. It's an urgent priority to find non-US alternatives to Azure, AWS, Windows, Cerner and Epic.

For clarity, I DO think we need proper standards for health and care. I think they need to be completely technically specified to be interoperable.

The way to do it is more like FHIR/openEHR collaboration or the Australian Sparked Initiative.

If the PRSB survive the next step must be total reform.
