(exception: the 4 therapists who worked with my dad before Medicare kicked him out of hospital rehab despite those therapists saying his care was medically necessary.)

You could also try a ukulele to start, to see if he likes it and enjoys the practicing. Much cheaper for a nice one and less painful on small fingers. And you can cheat with the tuning and tune it as a guitar so if he moves over to one the chord shapes are the same.
