They are run as a for profit trust me. I worked nonprofit the boards demand they operate like their businesses. Leaders are trained with Harvard business school management practices and so on. It’s all the same hellscape dying on an altar of efficiency.

I explained Medicare for all to my brother. Explained it would eliminate Medicaid, workers comp medical, va medical, all of the different insurance plans and so on and even as a somewhat right wing guy, but not a trumper, he was like yeah, do that.

He was pissed at me because it made him look bad and I need to stay out of it. I told him I would have if he wasn’t throwing someone else under the bus, which was what he was doing even if he didn’t realize it.

I ran the math on what pew research called a middle class income for a single person in Portland Oregon back in 2016. A very stripped down lifestyle with a bus pass, no car landed a middle class person $600 in the hole. This is with no leisure activities, not a cup of coffee. Nothing beyond basics.
