So I think it is best to delay. If the trainee is going to do first author work, they should have a good understanding of the complete process. Focus on the skill set and the rest will fall in place.

Thirdly, the training programs at my hospital require a graduation manuscript. Getting to the end of training with two incomplete projects makes it difficult to get a high quality manuscript if rushed, which leads to significantly more (and less rewarding) work at the back end

The time immediately after a presentation is the BEST time to write the paper. Trainee has recently prepped hard for presentation and knows their project and likely got some gentle redirections that can guide paper. All of these will fade with time and be harder in the future

I put the most value on manuscripts which requires to synthesize their project and reflect on the research project. Knowing all the steps helps the trainee guide their next project

Most of my trainees have limited research experience, so from our first discussion I emphasize that we’ll focus on the skills development. I want my trainees to have experience in study design, data collection, analysis, and presentations/pubs.

Excellent points!

From evaluating these prescribers, I can say fairly confidently that clindamycin prescriptions at our center were often associated with other poor prescribing practices:
-Prolonged courses
-Unnecessary indications (mild paronychia, prolonged post-op ppx after plastic surgery)

That is a fair point.

I wonder what the overlap is in the Venn diagram of early adapters of new brand name agents and the last people to stop using drugs with high risks of adverse effects (such as clindamycin)

Use of device in ECMO is experimental and temporarily reduction in several inflammatory markers (that may go up after 24 hours) is of unclear clinical significance.

More work is needed before I would offer to a pt outside of a trial.

Many shortcomings limit study’s generalizability:

-Rate of clearance actually lower than other studies (only ~20% of our Gram neg bacteremia has + FUBC)
-No mention of antibiotics used
-Many patients had circuit changes
-No effect on any pt based outcome
-F/u inflammation markers only 24 hrs
